Viewing your patient’s DxPortal chart can be done in three simple steps.
Step 1.
Ask the patient for their 8-digit “PatientChart Access Code”, located in the upper right hand corner of their DxPortal account.
Step 2.
Go to
Step 3.
Enter the 8-digit ‘PatientChart Access Code” that was given to you by your patient.
